Inmunología Clínica y Reumatología

Inmunología y Reumatología UC se caracteriza por un equilibrio entre un área clínica integrada por reumatólogos y un área de ciencias básicas formada por investigadores básicos en biología celular y molecular.

El Departamento participa en el comité de Doctorado en Ciencias Médicas de la Facultad de Medicina y en la presidencia y directorio de la Sociedad Chilena de Reumatología, siempre en pos del más alto estándar en atención, educación e investigación.

Historia

La creación de la sección de Reumatología se gestó en 1973, a la vuelta de Estados Unidos del Dr. Jacobelli después de terminar su entrenamiento en la Universidad de Chicago.

Cuando el Dr. Santiago Rivero regresó de su entrenamiento en México, en 1978, se conformó un pequeño laboratorio de la especialidad con el nombre por el de Inmunología Clínica y Reumatología para el estudio clínico e histopatológico de las enfermedades del tejido conectivo.

De ahí salieron varias publicaciones sobre aspectos clínicos y patológicos del Lupus Eritematoso y la Nefropatía Lúpica, sin olvidar aspectos clínicos y metabólicos de las artritis por cristales.

En 1981 se incorporó la Dra. Loreto Massardo y el grupo entonces alcanzó una masa crítica, pequeña, pero crítica al fin, que nos permitió en 1984 separarnos de las secciones de Nutrición y de Endocrinología para constituir un Departamento propio. Esto se hizo estimulados por los Drs. Pablo Lira y Gonzalo Grebe.

Tener un Departamento involucraba tareas mayores y así aceptamos en 1986 nuestro primer becario, la Dra. Carmen Rojas, manteniendo en los años siguientes nuestro aporte en la formación de becados de la especialidad.

En 1987 se incorporó el Dr. Alfonso González, doctor en Biología Celular y Molecular, con lo cual el Departamento adquirió una nueva dimensión.

A lo largo de los siguientes años, esta mezcla de clínicos y básicos demostró ser muy valiosa en nuestro desarrollo y es así como ahora, a casi 20 años de trabajo, tenemos un grupo sólido con nuevos investigadores incorporados y diferentes líneas de investigación que han producido publicaciones importantes tanto en el área clínica como básica en el ámbito de la Reumatología, Inmunología y de la Biomedicina.

Además, se mantienen proyectos de investigación financiados por fondos nacionales como Fondecyt, Fonis y otros.

Durante estos años este Departamento ha incorporado nuevos miembros aumentando su aporte en docencia, investigación, formación de nuevos especialistas y en una mejor atención para ofrecer a nuestros enfermos.

Sin embargo nuevos desafíos ya aparecen en el horizonte, nuevos proyectos se están gestando, el concepto de las enfermedades autoinmunes está en constante evolución, todo lo cual genera estímulos adicionales para un trabajo universitario más profesional, que sin duda el Departamento podrá enfrentar con éxito ya que emana de la generosidad y capacidad de trabajo de sus miembros.

Participamos en cátedras como Introducción a los estudios médicos y odontológicos (primero). El curso de Inmunología clínica se dicta en segundo y tercer año y  Semiología médica (tercero) con énfasis en semiología musculoesquelética.

La docencia en reumatología (cuarto año) incluye  las grandes enfermedades de la reumatología: Osteoartritis, Artritis Reumatoide, Fibromialgia, Lupus Eritematoso sistémico, otras Enfermedades del Tejido Conectivo, Síndrome de Sjögren, Espondiloartropatías y Artritis reactiva y  Vasculitis. La metodología incluye  clases magistrales combinadas con seminarios con aprendizaje basado en problemas en pequeños grupos de alumnos dirigidos por un tutor y sala espejo con inmersión de 2 semanas en reumatología. En quinto año hay seminarios en pequeño grupo en diagnóstico diferencial y profundización de los conocimientos adquiridos en reumatología.

Además mentorías en primero y cuarto año de Medicina. Participación en EUNACOM. Tenemos ayudantías para alumnos de pregrado en investigación, docencia e inmersión de verano.

Impartimos docencia en  Fonoaudiología, Odontología,  Kinesiología y Nutrición). Además médicos de nuestro departamento está a cargo de los cursos de Biología Molecular de la Célula Bio238M y Biología y Fisiología Celular, Bio 274E para Bioquímicos y Biólogos

Métodos

Grupos 10-14 alumnos. Seminario con formato de “aprendizaje basado en problemas”. Se plantea un caso clínico de un paciente con dolor articular, y los alumnos desarrollan las posibilidades diagnósticas y terapéuticas, guiados por un docente que actúa de moderador.

En cada sesión se realizará una evaluación escrita al inicio y al final.

La asistencia es de carácter OBLIGATORIO

Plantear diagnósticos diferenciales de dolor articular y lumbar, ajustados a la condición de cada paciente
Reconocer criterios diagnósticos clínicos, utilidad del laboratorio y radiología de las principales enfermedades reumatológicas

Específicos

Diferenciar compromiso articular mecánico del inflamatorio, y sistematizar número de articulaciones inflamadas y/o dolorosas en  compromiso mono, oligo o poliarticular.

Reconocer diferencias entre lumbago mecánico e inflamatorio, y  las principales patologías que se manifiestan de cada una de estas formas
Describir los síntomas asociados específicos de cada patología incluida en el diagnóstico diferencial del dolor articular y lumbar.

Plantear los diagnósticos diferenciales más probables, de acuerdo a los síntomas y signos de cada paciente que se presenta con dolor musculoesquelético.

Definir cuál o cuáles exámenes de laboratorio e imágenes se deben solicitar para confirmar las hipótesis diagnóstica

Reconocer la epidemiología y cuadro clínico de la Osteoartritis, Artritis Reumatoídea, Fibromialgia, Lupus Eritematoso sistémico, Síndrome de Sjogren, espondiloartropatías y vasculitis

Reconocer enfermedades sistémicas (neoplasias, hipotiroidismo, infecciones, entre otras)  que pueden presentarse como dolor articular

Evaluar la sensibilidad y especificidad de los exámenes de laboratorio general y los distintos autoanticuerpos para aproximarse al diagnóstico

Plantear un esquema terapéutico ajustado según la patología, y reconocer los principales efectos adversos de los medicamentos que habitualmente se utilizan en reumatología

Justificar terapias alternativas o complementarias necesarias (kinesiología, órtesis, terapia ocupacional etc)

Establecer un pronóstico funcional y de sobrevida en el paciente “problema”

Programa IV año Capitulo de Reumatología

II. Descripción del curso

En este capítulo se revisarán las principales enfermedades reumatológicas. Éstas constituyen un grupo heterogéneo de enfermedades que históricamente se agruparon como causantes de dolor músculo-esquelético no traumático y que tienen una etiología, patogenia y clínica muy diversa.

En el curso se enfatizará la integración y aplicación de los conocimientos entregados sobre: mecanismos patogénicos, manifestaciones clínicas y anatomopatológicas, epidemiología, métodos diagnósticos, tratamiento y complicaciones asociadas a cada enfermedad reumatológica.

Programa de postgrado en Inmunología Clínica y Reumatología

Desde 1984 hemos formado 40 médicos como especialistas en Inmunología Clínica y Reumatología de la Pontificia Universidad Católica de Chile quienes son exitosos profesionales distribuidos a lo largo de todo Chile y en el extranjero. Nuestro programa de formación dura dos años y está acreditado por la Asociación de Facultades de Medicina de Chile ASOFAMECH por el período 2015-2020. Los requisitos son ser médico chileno o extranjero con examen médico nacional EUNACOM  aprobado y especialista en Medicina Interna.

Investigación

En el departamento contamos con una vasta experiencia en investigación básica, con la participación de un grupo de investigadores capaces de interactuar con los clínicos, pero con una mirada enfocada en la búsqueda de los mecanismos celulares y moleculares, involucrados en el desarrollo y la mantención de patologías, no sólo autoinmunes, si no que también algunos otros ejemplos como el cáncer.

Esta investigación se ha desarrollado a través del tiempo con financiamiento, principalmente proveniente de concursos nacionales e internacionales. Además nuestros investigadores forman parte de centros de investigación mayores, como Milenio, Basal, etc.

Dentro de los temas de investigación de este departamento podemos destacar:

 

El tráfico vesicular de proteínas

Este tráfico está controlado por señales de destinación codificadas en la estructura de las proteínas cargo y por una maquinaria que genera vesículas de transporte, que luego se fusionan con compartimentos blancos específicos. Tanto la ruta secretoria a nivel de la red trans-Golgi (TGN) como los endosomas de reciclaje (RE) segregan proteínas hacia el polo apical o basolateral, generando y manteniendo el fenotipo polarizado de las células epiteliales. Recientemente, hemos dado evidencia definitiva acerca de un cruce entre el TGN y los RE, que opera en la segregación basolateral. Concomitantemente, resolvimos una controversia sobre el sitio de función del adaptador AP1B, mostrando segrega proteínas basolaterales exclusivamente en los RE que intersectan la ruta secretoria.

El Receptor del Factor de Crecimiento Epidermal (EGFR)

El EGFR constituye uno de los sistemas de control más ampliamente distribuidos de la proliferación y diferenciación celular, especialmente importante en células epiteliales. La endocitosis juega un rol crucial en la función del REGF. Después de la unión con el ligando, el EGFR dimeriza, su dominio tirosina quinasa es activado y el receptor activo se endocita. El EGFR internalizado y activo señaliza desde compartimentos intracelulares y finalmente su señalización se apaga por degradación en los lisosomas. Las células de carcinoma frecuentemente muestran una sobreexpresión y mutaciones oncogénicas del EGFR. Se han usado en clínica dos tipos de terapias basadas en drogas anti cáncer cuyo blanco es el EGFR: inhibidores de unión a ligando e inhibidores de la actividad tirosina quinasa. Su efectividad pareciera estar restringida a no más de 15-30% de los pacientes. Hemos desarrollado una nueva estrategia, induciendo la endocitosis del EGFR, lo que produce la muerte de muchos tipos de células tumorales que dependen de este receptor, haciéndolo una estrategia muy promisoria para el tratamiento de muchos tipos de cáncer dependientes de EGFR.

Auto-anticuerpos anti-P y NSPA

Los pacientes con lupus eritematoso sistémico (LES) frecuentemente producen autoanticuerpos neuronales, pero los blancos antigénicos y los mecanismos patogénicos de estos anticuerpos aún son poco conocidos. Recientemente determinamos que los anticuerpos anti-P ribosomal, que se asocian con lupus psiquiátrico, reconocen una nueva proteína integral de membrana de la superficie celular neuronal, que denominamos NSPA por Neuronal Surface P Antigen. En el cerebro, NSPA está preferentemente distribuida en áreas involucradas en memoria, cognición y emociones. También encontramos que los anticuerpos anti-P causan un rápido y sostenido aumento en el influjo de calcio en neuronas, lo que resulta en muerte celular por apoptosis tanto in vitro como in vivo. Nuestros resultados demuestran el potencial neuropatogénico de los anticuerpos anti-P.

Interacciones moleculares que controlan la sinapsis inmunológica entre la célula dendrítica y el linfocito T

Implicaciones en la defensa contra patógenos y tumores y en la prevención de la autoinmunidad. Nuestra línea de investigación se concentra en las interacciones moleculares que promueven la formación de la sinapsis inmunológica entre el linfocito T y la célula dendrítica presentadora de antígenos. Dicha sinapsis es esencial para la respuesta inmune responsable de proteger al hospedero contra los agentes patógenos y el crecimiento de tumores. Específicamente, estos estudios apuntan a dilucidar los mecanismos moleculares por los cuales la célula dendrítica controla la activación del linfocito T. Los nuevos conocimientos generados serán aplicados a la optimización de la inmunidad contra agentes infecciosos bacterianos y virales, así como también contra tumores malignos. Además, nuestro laboratorio intenta identificar los mecanismos moleculares desarrollados por los patógenos y las células tumorales para inhibir la formación de la sinapsis inmunológica, con el objeto de evadir la respuesta inmune. Finalmente, el entendimiento de los mecanismos moleculares responsables de la regulación de la sinapsis inmunológica es clave para interferir con la iniciación y desarrollo de enfermedades autoinmunes, tales como la Esclerosis Múltiple, el Lupus Eritamatoso Sistémico y la Artritis Reumatoide. En el laboratorio estamos desarrollando estrategias tanto de inmunoterapia como farmacológicas modular la sinapsis inmunológica a fin de evitar el desarrollo de autoinmunidad.

Algunas contribuciones significativas de nuestro departamento al conocimiento en salud y a mecanismos básicos involucrados en distintas patologías, obtenida a lo largo de los años se detallan en la sección publicaciones.

Atención

Nos especializamos en todos los tipos de artritis y enfermedades osteo-articulares y alergias. Nos dedicamos con mucho entusiasmo  a lupus eritematoso sistémico, artritis reumatoide, artrosis, artritis psoriática, gota, esclerodermia y a las enfermedades del tejido conectivo.

Atendemos  adultos y ancianos y acompañamos a las embarazadas con  algunas de estas enfermedades. Mantenemos una estrecha colaboración con   la   reumatología infantil para la atención continua de pacientes con enfermedades crónicas en el paso de  adolescente a adultos. 

Colaboramos con otras especialidades con médicos internistas, de medicina familiar, traumatología y ortopedia, dermatología, neurología y psiquiatría y nefrología. En reuniones  médicas periódicas discutimos casos de nuestros pacientes con especialistas de nuestro hospital en radiología oste-oarticular, biopsias, problemas respiratorios, nefrología, dermatología y  traumatología. Estamos vinculados con el servicio de Kinesiología de nuestra red de salud discutiendo pacientes caso a caso.

Nuestros  pacientes si lo requieren pueden recibir los más avanzados medicamentos biológicos  en  estudios de drogas en colaboración internacional con  agencias  como la Food and Drug Agency de EE.UU. y la agencia europea de medicamentos EMEA. Junto con nuestro equipo de reumatólogos clínicos, enfermeras universitarias especialistas en reumatología conducen estos tratamientos.   Esto se lleva a cabo en el  centro de investigaciones médicas CICUC. Los estudios cuentan con las aprobaciones de los Comités de ética de la nuestra institución  y del Instituto de Salud Pública ISP. 

Hemos incorporado la atención,  estudio y tratamiento de alergias.  Realizamos inmunoterapia. Hacemos desensibilizaciones a medicamentos para quienes requieran utilizar remedios y no los pueden recibir debido a alergia.

Contamos con el  programa “PROSPERAR” de atención integral dedicado exclusivamente a  pacientes con artritis reumatoide En el programa PROSPERAR prestamos atención conjunta por reumatólogos, traumatólogos, terapeuta ocupacional, kinesiólogos, psicóloga y enfermeras en reumatología.  Actualmente se atienden regularmente 605 personas en este programa.  Otro progama se dedica a pacientes con Lupus  Eritematoso Sistémico para los enfermos inscritos en el sistema GES de ISAPRES  que han hecho convenios con la red de salud UC-Christus (Cruz-Blanca, MasVida, Colmena). 

En nuestra clínica de artritis temprana tenemos la puerta abierta para atención rápida para quienes consulten en la red salud UC – Christus por síntomas iniciales de artritis en manos.

También se realiza el Programa PROSPERAR, un centro de excelencia en AR que cumple estándares de calidad en atención para personas que padecen Artritis Reumatoide. A este programa ingresan de preferencia pacientes de instituciones de salud previsional ISAPRES que mantienen convenios GES con la red de salud UC-Christus, y que son atendidos por un equipo multidisciplinario, entre las que se encuentran las Reumatólogas Dra. P. Burgos, Dra. M. Armstrong, Dra. A. Herrera, Dra. M. González, Dra. J. Durán, la Fisiatra Dra. C. Rigo-Righi, Coordinadora EU Nancy Vásquez. y la Terapeuta Ocupacional Pilar Ortega.

También se realiza el Programa Centro de Alergias UC, dirigido a niños y adultos donde se realizan exámenes, diagnósticos, controles médicos, inmunoterapias y desensibilización.
